Note : Any Caregiver that refers a new client to ACHA will have the opportunity to become a member of the Co-Op and both the client and Caregiver will receive benefits of lower bill rates through Care-credit Bundles and higher Caregiver wages & benefits.
Alaska Cooperative has established requirements that ALL providers (Caregivers) must complete before being paid to work for ANY recipient (Client).
1. Before attending orientation, providers should verify the recipient they will be working for has an active/open ACHA case (i.e. a Care Manager has completed an in-home assessment, care plan and has approved the case, and your recipient received ACHA paperwork). Orientation is for new providers whose recipient has an approved case and a case number. Please bring the recipient's case number to complete the enrollment paperwork and receive timesheets. If your recipient does not have a case number, ask her/him to look on their ACHA paperwork or ask that they contact ACHA by calling (907) 917-5515.
Orientation is for providers with new recipients, and their interpreters only. Please no recipients, children, family, or friends should attend. The required provider enrollment forms will be available and filled out at orientation.
2. To be admitted to orientation, all providers must bring an unexpired U.S. government-issued photo identification AND an original Social Security Card. Once again, you will need to have in your possession both a current ID (e.g. driver's license, state-issued ID card, U.S. passport, military ID...etc.) and your original Social Security Card. Applicants who do not have both ID and Social Security Card will not be admitted to orientation. Be aware, copies of the Social Security Card or other documents with a Social Security number will NOT be accepted.
3. Complete and pass a Alaska Department of Justice (DOJ) criminal background check. To complete the background check, you will need to be fingerprinted via LiveScan. Further detail is available through HR
